Does anyone have a working feedback form?

I need to find out where and how to put the email address of where the submit should go.
Can anyone help?

You can make a nice Feedback Form at the following site:

Feedback Form Maker

Requires two files. Your normal html file designed to your site's layout and the *.php file that is called. Both reside in the same directory so no special PHP problems arise.
