Making User Choices affect Form Options

Does anyone know how i can make the choices a user makes affect the options available further down a form? For instance, if I was making a form for people to advertise on my site, and they had selected three pages they wished to advertise on, how could i make it so that urther down in the form they were asked to upload three adverts?

Any help much appreciated
