well, ill go ahead and say that i have no idea about the codes. im sure there is somebody out there to help you. however, when i do polls for my website, i end up just using a pre-programmed service such as http://surveymonkey.com . they have a free service, and you can create your own questions to print or have on the net. however, they are not embedded into your site; you have to click on a link to get to them. but you can tell the surveymonkey site to givce you a code which will create a pop-up window to take people to oyur iste. other ways might include just using forms, but i can never get those to work. HOPE THIS HELPS : )

ok. dont use the surveymonkey site. i ofund a better one. thsi site has all the features of surveymonkey, but they are absouloutly free. htere is a paid account availiable, but you dont need it;, the free has all the same features, but just a samller storaage space. if your website would have more than 100 people take the survey per month, then subscribe to the free account. there is a sale in May, so hurry if you wnat it. as far as the codes, it will give you a code to embed in your site for free unlike survey monkey.
